Community Care Access Centres
Your Community Care Access Centre (CCAC) offers professional support services in your home.
You may be able to have one or more of the following services:
- Nursing
- Nutrition
- Physiotherapy
- Occupational Therapy
- Social Work
- Speech Therapy
- Home Support, when personal care is required
You may be able to have one or more of the following support services:
- Drug card
- Laboratory services
- Medical equipment and supplies
- Transportation for medical or therapy appointments, only when no other options are available
Requests for Community Care Access Centre services can be made by any person, including you, the patient.
The case manager at the Cancer Centre can coordinate services with any Community Care Access Centre in Ontario.
All services are paid for by OHIP.
