Your Symptoms Are Important To Us!
Edmonton Symptom Assessment System (ESAS)

We want to know how you are feeling each time you come to the Cancer Centre for your appointment. Only you can tell us about your symptoms. By telling us about how you are feeling at each visit, your healthcare providers at the Centre will be able to assist you with your symptoms. The Edmonton Symptom Assessment System, or ESAS, will provide us with information about your symptoms.

What is ESAS?
ESAS provides a review of how you are feeling based on nine different symptoms.

Why should I complete my ESAS?
You and your clinic healthcare providers are partners in your care. Within that partnership, you will be able to help us understand how you are feeling and we will be able to identify problems and offer help as soon as possible.

When do I complete my ESAS?
You complete your ESAS before each appointment at the Cancer Centre.

How do I complete my ESAS?

  • When you check in the receptionist will give you a barcode card.
  • A volunteer will show you where the kiosk (computer station) is located. There is a computer in each of the waiting rooms A, B, and C.
  • NO computer skills are required.
  • The volunteer will show you how to use the touch screen computer and will be available to assist you.
  • You will answer nine questions by tapping the computer screen. When you have finished answering the questions, your results will be printed right at the computer.
  • Give your printed results to the healthcare provider you see at your appointment.
  • Please keep your barcode card for use at your next visit.

Your symptoms are important to us. Only you can tell us how you are feeling. Please see a sample page of ESAS below.
