Community Home Nursing

Community home nurses are managing complex patient situations and have a greater responsibility for cancer symptom management and education on self-care strategies since cancer care is being shifted from the inpatient to the outpatient setting (Fieler & Hanson, 2000).

Community home nurses function as specialized oncology nurses when providing care to patients and families and they share their specialized knowledge with family physicians when discussing plans of care. They also provide an important link and unique role with respect to coordinating the continuity of patient information and care between primary care settings e.g. family physicians and health care facilities.

Providing care in the home can be challenging for the nurse when needing to interact with the patient and family within their environment on their terms because it is the patient in charge of the environment not the healthcare provider that is seen in hospital settings.
